Instructions
After using an outside cut for the overall shape and center hole, you will likely need to cut the open kerf with a handsaw, as I have done. The 3mm holes for the leveling screws will need to be done with a drill bit, unless you have a bit for your shaper that can accommodate such a small size. Use a countersink to recess the head of the cap screw on the top. I borrowed the hardware from my factory plate, but you can divine some of the hardware based upon the uploaded part schematic. NOTE: Recommend cutting with grain running the length of the plate (unlike the one on the left side of the second photo).
