Older bandsaw insert was looking a little beat so I decided to make one out of .15" thick ash I had resawn and planed. Made sure to create an open design for dust collection air flow. File is bit and depth encoded. This design is for a Jet 14" Pro (105" blade) and up to 3/4" depth blade which is the max for this machine.

Stock 1/8" bit
1/16" bit
.15" thick hardwood veneer
Keep in mind you are cutting the back side of part so place the nice side face/tape down: Tape down a piece of veneer to Workstation Shelf. I used .15" thick and a 6" x 6" piece so I could lay out and cut 4 parts at a time Set up and z-touch the 1/8" bit Cut the inner perimeter "D" shape as outside cut to .054" depth with zero offset Set offset to .1" and do same "D" shape Offset to .2" for one last pass of "D" to clean up edge Change offset back to zero Pocket cut of the top rectangle at .054" depth Set rectangle to inside cut and proceed at .054" depth Cut middle slot at .06" with zero offset [optional] Cut all of the holes with helix turned on to .19" depth and zero offset Switch bits to 1/16" and z-touch Cut the outside perimeter of the part at depths of .063", .125" and .19". You need the 1/16" tool to make the notch the right size for the indexing roll pin Remove the part and sand the edges. To clean up burrs inside the hole, a light touch with #11 Xacto hobby knife works great. If the burr doesn't come off easily, reverse direction. Doing it this way makes fairly quick work of cleaning up the edges. Final step is to lightly draw a center line for your bandsaw blade on the top edge of the solid part of the insert. Make sure there is an existing insert in the bandsaw and turn it on. Slowly cut the pencil line with bandsaw keeping an eye on when the two inserts are lined up with each other, then carefully back out the insert you are cutting and you are done. You may need to sand the face a little to make sure it is flush with bandsaw table. If your stock is a little thicker than .15" no worries, just add the difference to all of the depths. You could probably use material up to .2" thick.
